SRA Waiver and Registration Form

In consideration of the Participant(s) being allowed by SwimRight Academy (SRA) to participate in and/or attend any and all activities associated with the SwimRight Academy (SRA). I hereby agree to waive any and all claims, release, discharge, hold harmless, indemnify, defend, and covenant not to sue, for myself, my heirs, executors, administrators, and assigns, LK Aquatic Services Inc. DBA as SwimRight Academy, deToledo High School, its officers, directors, employees, agents, volunteers and members from and against any and all claims or demands due to or arising from bodily injury, personal injury, illness or death, as well as any and all property damages sustained of any nature which might be incurred by me and/or the Participant while participating in said activities. By signing below, the Participant (or parent/guardian if the Participant is a minor) acknowledges and accepts the risks of physical injury associated with participation in the activity described above. The Participant (or parent/guardian) accepts personal financial responsibility for any medical expenses, bodily injury, personal injury, and property damages sustained during on in any way connected with or related to the activities. I knowingly and fully assume all risks, for myself and my child/ children, associated with participation.

The undersigned agrees to abide by all rules and conditions of participation posted or explained by the Lifeguards and employees at the SwimRight Academy.



SRA POLICIES:

  • My initial payment includes a registration fee of $55 per child and includes a cap and Student Progress Book. Parent-Tot registration is $45. An annual registration fee of $15 per child, $35 per family (3 or more students) is due on my anniversary.
  • While we strive to do so, SRA cannot guarantee the same instructor for each lesson. If my instructor is absent, another qualified instructor will be assigned whenever one is available. If possible, I will be notified of such changes but SRA cannot guarantee to do so.
  • I understand that there is no free swim time during lessons. Students enter the water when called by their instructor and exit at the end of their lesson. There is no lifeguard on duty.
  • I understand that although my child may pass certain skill levels during their classes, children need supervision around or near water at all times and it is dangerous for me to assume that any child is totally ‘water-safe.’
  • I understand that there are risks involved with swimming and related activities. I hereby agree to assume all liability for myself and my children while at SRA, and I further agree to hold harmless the officers of SRA, dTHS and their employees for any complications or injuries that may result from myself or my children attending LKSA.
  • I authorize SRA / dTHS to use my contact information to communicate any schedule updates and announcements. We will never share any client information with outside parties.
  • I am aware that parking is shared with other dTHS programs and is available on a first come, first served basis. I will allow enough time to park in case of congestion.
  • By entering the Aquatic Center, I consent to be photographed, videotaped, filmed, audio recorded or recorded in any other manner. My presence here acknowledges that such photographs, film, video, audio recording or any forms of recording of me and my family may be used by SwimRight Academy, and/or by its successors, assignees and licensees for marketing and promotional purposes only.



MAKE-UP LESSONS:

  • I understand that lessons must be cancelled by 9:00am the day of the lesson to qualify for a make-up
  • Make-up’s are limited to one per month and are valid for one calendar month from the date of the cancellation, as long as I am currently enrolled. 
  • If SRA is unable to provide a substitute on the day of my lesson, I will be provided a FREE make-up lesson that is good for 30 days from the cancelled class date.
  • Once scheduled, make-ups cannot be rescheduled.



TERMINATIONS:

  • I agree to be enrolled in the program for two months before submitting a notice to terminate lessons. 
  • I agree to give SRA 30 days advance notice to terminate my student's lessons. I understand that I can do so by completing a Lesson Termination Form which is available from the swim academy office or emailing info@swimrightwh.com. I will include a message with my child/children's name, date of birth, day/time of swim lesson and the specific wording "termination of swim lessons."
  • My final bill will be prorated to pay for lessons up to my termination date.
  • SRA does not offer refunds for any unused classes.



MONTHLY BILLING:

  • I understand that I am enrolling for continuous swim lessons and that my bank account or credit card will automatically be charged a set monthly fee for all lessons my child/children are enrolled in during the billing month.

  • Charges will be made on the 25th of the month (or the first business day thereafter) for the following month.

  • If I enroll in classes after the 10th of the month, my initial payment will be prorated for the current month in addition to fees for the following month.


  • Billing is automatically prorated during months when SRA has planned closures.

  • SRA does not offer refunds; however, if I am not happy with any lessons, I will notify the office on the day of the lesson and be provided with a FREE make-up lesson.
