ACKNOWLEDGEMENT OF RISK, ASSUMPTION OF RISK AND RESPONSIBILITY AND RELEASE OF LIABILITY

In Consideration of being allowed to enter the play area and/or participate in any party and/or program at Rough & Bumble KC, the undersigned, on his or her behalf, and on the behalf of the participant(s) identified below, acknowledges, appreciates and agrees to the following conditions:

I represent that I am the parent or legal guardian of the participant(s) named above, or I have obtained permission from the parent/legal guardian of the participant(s) named above to execute this agreement on their behalf. I agree that the participant(s) named above and I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation in any party and/or program at Rough & Bumble KC. In addition, if I observe any hazard during our participation, I will bring it to the attention of the nearest Rough & Bumble KC employee or official immediately. I understand that I am responsible for my child’s safety and behavior at all times each visit.

I am aware that there are inherent risks associated with participation in Rough & Bumble KC programs, parties and I, on behalf of myself and the participant(s) named on the reverse, knowingly and freely assume all such risk, both known and unknown, including those that may arise out of the negligence of other participants; and,

I, for myself and the participant(s) named above, and our respective heirs, assigns, administrators, personal representatives, and next of kin, hereby release and hold harmless, Rough & Bumble KC, LLC, and Rough & Bumble KC management, their affiliates, officers, members, agents, employees, other participants, and sponsoring agencies from and against any and all claims, injuries, liabilities or damages arising out of or related to our participation in any and all Rough & Bumble KC programs, activities, parties, and the use of the play area.

I HAVE READ THE FOREGOING ACKNOWLEDGEMENT OF RISK, ASSUMPTIONS OF RISK AND RESPONSIBILITY, AND RELEASE OF LIABILITY. I UNDERSTAND THAT BY SIGNING THIS DOCUMENT, I MAY BE WAIVING VALUABLE LEGAL RIGHTS.

Photo & Video Release Form

I hereby grant the Rough & Bumble KC permission to use my likeness in a photograph, video, or other digital media (“photo”) in an y and all of its publications, including web-based publications, without payment or other consideration.I understand and agree that all photos will become the property of the Rough & Bumble KC and will not be returned.I hereby irrevocably authorize the Rough & Bumble KC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge the Rough & Bumble KC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY THEIR SIGNATURES BELOW.
