Tattoo Consent, Waiver & Release Agreement for Solana Studios  

I acknowledge that I have been fully informed of the potential risks associated with receiving a tattoo. These risks may include, but are not limited to: infection, scarring, allergic reactions (to pigments, latex gloves, or soaps), and difficulties in detecting skin conditions such as melanoma. I understand that, despite all safety precautions taken, both known and unknown risks may still occur. By signing this form, I freely accept and assume all risks related to the tattoo procedure and application.

I fully release and discharge Solana Studios to the maximum extent permitted by law, from any and all liability—whether arising from negligence or otherwise—for any injury, loss, or damages, including consequential damages, that may result from the tattoo procedure. This waiver applies to myself, my heirs, estate, executors, and assigns.

I affirm that I have had the opportunity to ask questions about the tattoo process and that all my questions have been answered to my complete satisfaction. I have received detailed aftercare instructions and understand that proper aftercare is crucial to prevent complications. I acknowledge that failure to follow these instructions may result in infection or fading. If a touch-up is needed due to my own negligence, I understand I will be responsible for the cost.

I confirm that I am not under the influence of alcohol or drugs and that I am voluntarily choosing to be tattooed without any form of coercion.

I further confirm that:

• I do not have any medical conditions that may interfere with healing, including diabetes, hemophilia, epilepsy, heart conditions, or any immune-related disorders.

• I am not currently taking blood thinners.

• I am not pregnant or nursing.

• I am not undergoing chemotherapy, radiation, or organ/bone marrow transplant without first consulting my physician and following their prescribed antibiotic protocol.

• I do not suffer from any mental impairment that would affect my judgment in consenting to this procedure.

I understand that:

• Solana Studios is not responsible for the meaning, accuracy, or spelling of symbols or text I have chosen.

• Variations may occur between the chosen design and the final tattoo as applied to my skin.

• Tattoo pigment may fade or change over time due to sun exposure and natural pigment dispersion beneath the skin.

• Tattoos are permanent and removal may be difficult, expensive, and may not fully restore my skin to its original state.

I understand that Solana Studios maintains a strict NO REFUND policy, and I agree not to request or demand a refund under any circumstance.

In the event of any legal action, I agree to reimburse Solana Studios for all attorney’s fees and costs should the artist or studio prevail. I agree that any legal proceedings shall be held in Larimer County, Colorado, which shall have exclusive jurisdiction.

I confirm that I have been provided adequate time to read, understand, and consider this agreement. I understand this is a binding legal document and that I am waiving certain legal rights, including the right to recover damages.

If any part of this agreement is found to be invalid or unenforceable, the remaining provisions will continue in full force and effect as if the invalid portion were not included.

I certify that I am at least 18 years old, have presented valid identification, and am legally capable of entering into this agreement.

BY SIGNING BELOW, I CONFIRM THAT I HAVE READ AND UNDERSTOOD THIS AGREEMENT AND VOLUNTARILY AGREE TO BE BOUND BY ITS TERMS.
